5. You did'nt understand the problem or scope out the specific issues ended up answer the wrong question
6. You did'nt ask the other person what had already tried
7. You did'nt ask the other person if they have any doubts or reservations about advice given
8. You gut instinct is telling you that something is off but you did not ask questions to validate/invalidate your impression
9. You did'nt follow-up to see if your advice was useful
10. You gave unsolicited advice
