Ten Pieces Of Data Needed For Subsidies
Not motivated today... so we'll go with something simple.
Our clients (developmentally disabled adults) get housing subsidies from the State based on 30% of their income (almost exclusively Social Security).
1. Name
2. State Medicaid ID #
3. Date of Birth
4. Social Security Number
5. Legal Guardian Name, Address, E-Mail, Phone
6. Group Home Location
Including address, State program ID#, group home manager contact information.
7. Group Home Owner & Information
Which is our agency, but others do lease buildings for their clients. Includes primary contact, address, phone, e-mail, and tax payer ID number.
8. Client Gross Income & Source(s)
9. Client Representative Payee
Who manages the client's funds, which usually is a parent, the agency, or an agency providing rep payee services.
10. Calculation of Client and Subsidy Amounts
30% of clients income, the State funds the difference. And despite using their Excel sheet to calculate the split, the subsidy letter is almost always a few dollars different.
